POSITION SUMMARY
This position has primary responsibility for driving the facility vehicle and, when needed being the CNA used in transporting residents outside both to and from the facility. This position will require a valid non-restricted Michigan Driver’s License with a Chauffeur’s endorsement (a restricted license is a prohibited liability under the Facility’s vehicle insurance). This position requires current CPR Certification and additional other specialized in-service training may be required from time to time. Working forty (40) hours per week as assigned/daily hours may fluctuate. Working in Central Supply will be used as fillable hours when not in need as a Driver or a CNA. Central Supply Clerk would perform routine manual and detailed clerical work to include receiving, storing, stocking and inventory duties within Central Supply.
PERSONAL QUALIFICATIONS
I will hold a Chauffeur’s License in the State of Michigan or the ability to obtain a Chauffeur’s License upon hire. I am able to take verbal direction, and able to make good independent observations. I am able to cooperate with my peers and team members from other departments. I have good interpersonal communications skills with residents, families and licensed nurses. I am patient, reliable, empathetic, tactful, adaptable, self-motivated, and capable of projecting a positive public image and I am able to maintain resident confidentiality. I am detail oriented, and have the ability to multi-task and work independently. I have the ability to interact with staff at all levels in a fast-paced environment. I present a professional image to employees and vendors. I am a High School graduate or comparable education. I am able to work flexible hours to accommodate residents scheduled appointments. Must be at least 18 years of age and in good physical and mental health. Must be able to read, write, and follow oral and written directions. Must have a willingness to work with other departments and supervisors. Must be in good standing, defined as having no corrective actions (or pending) within the last 12 months at greater than a verbal warning level.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Primary responsibility for driving facility vehicles used to transport residents. Also performs deliveries and pick-ups outside the facility as assigned. Driver must be able to assist in transferring residents.
- Performs and documents completion of vehicle mileage log and checklist before leaving for every transport. In addition, maintains clean exterior and interior of facility vehicle(s).
- Assures that all resident wheelchairs are properly positioned and locked down in the vehicle being used, as well as ensures that all wheelchair brakes are locked properly, assures seatbelts/ shoulder straps are properly applied to all residents.
- All residents’ appointments will be scheduled through the Nursing/MDS Secretary in which, will notify the driver/ transporter via telephone, pager or in writing.
- All requests (supplies, mail, pick-up or delivery) will be scheduled through Nursing/MDS Secretary.
- Must submit to an annual physical with vision screen, by the Medical Director (or designee). Additional testing may be required, if recommended by the MD.

All procedures or other job-related tasks that involve an inherent potential for mucus membrane or skin contact with blood, body fluids or tissues, or a potential for spills, splashes of them.
COGNITIVE AND SENSORY REQUIREMENTS
Talking: Used to communicate with others.
Hearing: Needed when taking instructions and listening to proper functioning and timing of machinery.
Sight: Used constantly in performing tasks.
PRIMARY PHYSICAL REQUIREMENTS
Lift up to 50 lbs.: Occasional required when transferring a non-ambulatory resident weighing between 100 and 160 pounds. Two aides are typically utilized when resident is unusually heavy or combative, or frequently required when lifting a resident, or in accordance with the resident’s care plan. Regularly required to lift all product/supplies that arrives at the facility.
Reach above shoulder height: Regularly occurs when reaching for medical records or supplies located on shelves.
Push/Pull: Regularly required to push a wheelchair which rolls easily across tile or carpeted floor.
