What’s Next?

Once you are approaching the top of the waiting list, we will contact you to see if you are still interested in transitioning to Canal View. Should you accept, the Admissions Coordinator will set up a date and time to make the transition (typically within five days of accepting).

Upon accepting admission to the facility, you will be asked to provide the following:

• Social Security card
• Medicaid card
• Medicare card
• Medicare Part D provider card
• Any other insurance cards
• Guardianship/DPOA forms (if applicable)
